• Proof of Age ( Birth Certificate)
• Enrollment Application
• Previous school report card (if applicable)
• Medical History Form
• Copy of Immunization Records
• Copy of passport – (if applicable)
• Residence Card – (if applicable)
• 1 recent passport sized photo
• Student interview
• CSB placement test (if applicable)
• Registration fee received
